Polish chips off the ends of the nail tips because of wear and water. The more you do and the more you wash your hands, the more likely the polish will chip. Unfortunately, there isn't a completely chip-proof polish on the market yet, but there are a few things you can do to make the polish last longer (and it sounds like you are taking very good care of your nails). First, apply a new coat of top coat every other day. This alone can increase the life span of your manicure for a week longer than usual. Second, if you do dishes or other work that requires you putting your hands in water, wear rubber gloves; water causes the natural nail to expand while the polish does not, hence the chipping. And third, learn to do things with the pads of your fingers (typing, dialing phones, pushing buttons), rather than the tips.
I reviewed OPI's Chip Skip on this board not too long ago, and I think it's a good product that definately does reduce the amount of chipping in nail polish, however, it has been my experience that there isn't much you can do about that little sliver at the very tip of the nail that polish tends to wear off first. The top coat every other day trick seems to work the best.
